Golf Event for Leukemia Research in Memory of

Fees:

Foursomes: $280 with cart

Individuals: $70

Includes golf, cart, meal, donation and prize money.

 

Note: Carts with roofs will be allocated in the order registrations are received.

 

Please invite spouses, families and friends to join us for a casual meal, silent auction and drawing of door prizes at 5pm. Cost of the meal for non-golfers is $10 (Adults) - $5 (Children).  The meal is included for golfers.

*$15 donation per person recommended to be eligible for door prizes. The more you donate the more chances to win!

Matching Grant Information

If you are employed by a company such as IBM that is in involved with a Matching Grant Program and you would like to participate, that would be great! The donation to leukemia research must be at least $25.00 to be eligible. If you wish to participate, forms will be available to sign the day of the tournament. Write two checks: one should be made payable to Mayo Foundation for the donation amount and a second check to Judy Lundy for the golf amount.
